Photocopy of birth Certificate, result of your last year’s results (for Grades 1-7
only), three passport size photographs and one family photograph (parents &
siblings only) |

School’s Copy of “Undertaking & Addendum to undertaking” initialed on each page
& signed by both parents. |
|
|
|
Transport Routes copy marked by your preference of stop (or else with a request
for a new stop in case your area is not covered – refer to General Instructions
below for more details. |
